Please be advised that more information may be needed by our office; such as your auto insurance card, personal health insurance card, and police report if available. Contact our office for further details.

Workers’ Comp:
When filing for workers’ compensation, make sure that you have reported the accident to your employer. In some cases you may be required to have one initial visit with the Comp carrier’s designated physician before being seen at our office. Be aware that treating with that physician more than one time will classify them as your referring doctor, in which case you will need a referral from that doctor to be treated at our office.
